Please see my referral form, below, which is required if insurance will be billed. 

I am currently in-network with Blue Cross and Blue Shield of Texas, United Healthcare, Medicare, and Superior Healthcare. 

Traditional Medicare covers visits with a Registered Dietitian (Medical Nutrition Therapy) for the following diagnoses only: Diabetes, Chronic Kidney Disease Stages (without dialysis), or up to 36 months after Kidney Transplant. 

Patients should call insurance provider to confirm coverage for a particular diagnosis.
